  • Ensure Refrigerator is Stocked Upon your Return from the Hospital or Rehabilitation Center
  • Pick up or Drop off of Prescriptions
  • Obtain Post-Operative Medical Devices (walkers, garments, compressions)
  • Coordinate Companions, “In Home Care” or Private Duty Nurse to Distribute Medication, Help with Bathing or Dressing
  • Serve as a Liaison for a Client’s Family with In-Home Support Services
  • Arrange Travel for Follow Up Visits to the Doctors
  • Keep Family and Friends Informed About Medical Issues if Desired

  • Referrals for Maintenance/Repair/Improvement Professionals
  • Referrals to Elder Lawyers, Financial Services, Home Care Services, and Other Service Professionals as Needed
  • Referrals for Housekeeping/Cleaning Services
  • Gather and Present Estimates for Service Professionals

Relocation Support

  • Assist Clients in Selecting and Arranging for Alternate Living Arrangements
  • Relocation Coordination
  • Relocation Support (address changes, drivers license, voter registration, bank setup, mail forwarding, phone, utilities etc.)

  • Facilitate “Keeping In Touch” with Family through E-Mail and VideoChat Capabilities
